appropriate form, complete, and bring to your initial appointment:

New Patients

BrainCore Therapy Patient Forms:

All new BrainCore patients are required to complete the following intake forms:

• Acceptance Application & Informed Consent (Adobe PDF -- 2 pages)

 

ADHD Patients:

In addition to the three aforementioned intake forms, please complete the Vanderbilt Assessment Scale as well as the symptoms checklist for either Pediatric or Adult ADHD:

• Vanderbilt Assessment Scale
• Pediatric ADHD: Self Report Scale: Symptom Checklist (Child)
• ADULT ADHD: Self Report Scale: Symptom Checklist (Adult)

Headache Patients:

Please complete the three intake forms as well as the two forms listed below:

• Headache Disability Index
